Self-pay rates

Insurance accepted for most services. Rates below apply when insurance is not used.

Practice policies

Cancellation Policy

  • Cancel or reschedule at least 24 hours before your appointment to avoid a fee.
  • Cancellations under 24 hours or no-shows are subject to a $75 late cancellation fee.
  • The fee is charged to the card on file and is not covered by insurance.
  • Repeated no-shows may result in discharge from the practice.

Payment

  • Payment is due at the time of service.
  • We accept all major credit and debit cards, HSA, and FSA.
  • We do not accept cash or personal checks.
  • Outstanding balances may pause future scheduling.

Insurance

  • We are in-network with Aetna, BlueCross BlueShield, Cigna, UnitedHealthcare, Medicare, and Medicaid.
  • Co-pays and deductibles apply per your plan — verify benefits before your visit.
  • Superbills provided for out-of-network reimbursement upon request.
  • Self-pay rates listed above apply if insurance is not accepted or not used.

Intake Paperwork

  • All new patient intake forms must be completed at least 24 hours before the appointment.
  • Incomplete paperwork may result in rescheduling.
  • Forms are sent securely via the patient portal after booking.
  • Bring a list of current medications to your first visit.

Communication

  • Use the patient portal for all clinical questions, medication requests, and follow-ups.
  • Non-urgent questions receive a response within 1–2 business days.
  • Do not use email or voicemail for clinical concerns — use the portal.
  • Email info@unityhealthandwellness.com for scheduling and general administrative questions.

Crisis & Emergency Policy

  • Unity Health & Wellness is NOT a crisis service.
  • Do not use text, email, or voicemail for urgent or life-threatening concerns.
  • If you are in crisis, call 988 (Suicide & Crisis Lifeline) or go to your nearest emergency room.
  • Call 911 for any immediate danger to yourself or others.
